Scoring Key:
5 or more items that are scored with "4's" and "5's" are significant.The individual should seek more in depth testing.
3 or 4 items that are scored with "4's" or "5's" are suspicious. Further testing should be considered.
If more than half of the items have at least a score of "3", the diagnosis should be at least ruled out. Testing should be considered.
